Executive Summary
Cerveceria India, Inc. is an established brewery in Mayaguez, PR, with 280 employees. The company is committed to comply with all applicable requirements for its beverage manufacturing operations. Cerveceria India uses one of the compounds which appear on the list of Risk Management Planning chemicals and which is stored in greater-than-threshold quantities. The facility operates an ammonia refrigeration system as part of its manufacturing operations. This Cold Block refrigeration system utilizes ammonia as a refrigerant in a closed system. *Prevention Program: Cerveceria India, Inc., has implemented a prevention program for the covered process. Hazard reviews have been conducted, and updated at least every five years to aid in identifying hazards and recommendations for improvement. Operators and maintenance personnel are trained and provided with written instructions as well as safe work practices. Training is provided at least every three years or more often if necessary. Contractors working in or adjacent to the refrigeration system receive training in applicable ammonia hazards and safe work practices. Cerveceria India, Inc. has developed procedures for maintaining the ongoing integrity of equipment. All incidents and near misses are investigated and recommendations resolved as soon as possible.

*Five Years Accident History: The facility had an accidental release on October 6, 2007. A release of carbon dioxide and ammonia from Cerveceria India, Inc. located at Mayagez, Puerto Rico, resulted in the evacuation of residential population within a 2 square mile area (2,000 people were e
vacuated in total). One employee and 36 residents required medical attention and/or hospitalization. Seven vessels, part of a carbon dioxide transfer system, failed and the resulting overpressure impacted an ammonia refrigeration system. The system failure resulted in a release of 85,000 lbs. of carbon dioxide and 18,074 lbs. of ammonia over a 3.5 hr. period.

*Emergency Response Program: Cerveceria India, Inc., has developed and emergency action plan to respond to minor fires and spills and for a safe plant evacuation. Cerveceria India, Inc. has coordinated emergency response through a private contractor specialized in response for this type of emergencies. Cerveceria India, Inc. is actually participating in the LEPC meetings and is committed to contribute to this program. *Planned Changes to Improve Safety: As part of the incident investigation, 23 recommendations were identified. Training and a Process Hazard Analysis of a new CO2 system have been completed. Also, improvements to the Emergency Response Plan and related equipment are being pursued.

Prevention Program Description
The covered process starts with a liquid ammonia Receiver, located at the Compressor Room. From this Receiver ammonia is transferred to an externally located Separator. Liquid ammonia is pumped from the Separator to several end users, such as air coolers, vessel jackets, plate heat exchangers and air coolers. Evaporated ammonia is suctioned back to the compressor units, followed by condensation at the evaporative condensers located at the top of the Compressor Room. Condensed ammonia is then collected back at the Receiver.
